Vascular Referrals / Consultations
If you fill out the vascular referral form below, we would be happy to quickly see your patients in consultation. Ultrasound walk-ins are welcome.
We accept Medicare, Medicaid, and all Major Insurance Plans
If you are a physician, doctor’s office, or other healthcare facility that is interested in setting up an appointment for one of your patients, please print out the form below and fax it to our Vascular Center. Our office will then contact your patient within 24 hours to arrange an appointment time that is convenient for them.
We have appointment available Monday – Friday from 8 am to 5 pm. Ultrasounds and segmental pressure studies are typically able to be performed on the same day as their initial consultation if the studies are needed. We do have bilingual office staff who are fluent in Spanish.
